I had a Telecaster and a Strat body setup from scratch with NNG Guitars, which is run by a Thai luthier, Prasit.
He is accomodating, his pricing is reasonable, you can even supply your existing set of pickups and he is able to procur some of the well-grained woods, if thats your interest.
Alternatively, just get a 2nd hand guitar you like and mod accordingly (as 3noteAbar suggests).
US custom shops do a wonderful job as well on instruments. The extra $ you pay are for the higher workmanship cost in generating a (normally) higher level of finishing/craftsmanship.
You buy first hand purely because you are looking at a customised keeper guitar (no intention to sell), else you should just get a 2nd hand mint version.
However, if you dont know exactly what you want in a customised instrument, you'd be paying for a costly lesson, lol.
