In-ear earphones, are generally used to isolate external noise from the surroundings so you can enjoy that music from your player much better when you're commuting in the train daily.
With regards to loss of hearing, when using in-ear earphones, you shouldn't bring the volume to a level you'd normally listen to when using normal earphones because that simply defeats the purpose of having in-ear earphones to block off external noise.
