Advice on your survey design:

1. Allow multiple answers for some questions (eg. Q1)
2. A scale (such as the Likert Scale) will be better for Q6. "Very important" and "not important at all" are two extreme ends of a scale.
3. Demographics will help you analyse your results better (e.g gender, age etc).
