Pitch problems?

Hey guys. Okay. Took me a while to figure out,cos I was reading a certain thread about this too. It concerns the pitch of each and every note of the guitar. I'm using an SX Strat standard. Before 2006 issue.
I gave it a setup early this year during Jan at Beez's. Did the fretwire,pu's,+ blah2. But the only thing I didn't know was the pitch of the notes. Okay Here's the question.
I fret the 5th fret of the B string. Yes,it sounds the same as the open string of the first string. But if you listen closely. They do not exactly sound the same,and may tend to be flat or sharp? Its an example. Its the same as starting to solo on the twelth fret using the E minor pentatonic of the first string. 12,15,17,19,21. As I start to fret,the solo sounds outta tune or slightly flat. What is causing this problem? And btw I use Daddario's 11 - 49's (Jazz) .But when I pluck each string,the tuner states that my strings are in tune. How do I fix this problem of getting each and every note in tune? Or is it impossible? Is it because I have a slightly heavier gauge?
